Thank you for enquiring about registering with the Medical Centre which is the branch surgery of the Underwood Surgery. If you are due to start university in September and are living in Cheltenham please use the form which you have been emailed, or access it here; then print it and bring it fully completed to registration. If you do not register when you arrive at University you will need to register in person at the Medical Centre with the above form and your Student ID card as we need to confirm your identity. Advice for completing form If you have been fully registered with a GP in the UK before, we will need your NHS number for section 8. As it says on the form, this is not your National Insurance number. It is a 10-digit long number and does not contain any letters. If you do not know it and do not have any hospital letters etc. with it on, your current GP surgery will be able to help you. If you are moving within the UK, please ensure that you are also able to give us your previous address, including your postcode. We will need this in order to trace your medical records. If you have never registered with a GP in the UK before then please ensure that you will be able to provide the full date of entry to the UK. We must check all forms carefully so please allow for the time this will take when you choose to come to attend.
